New Client Information
Our practice is located in Chicago’s Lincoln Park neighborhood. Our clinic address is 1925 N. Clybourn Avenue, suite 300, Chicago.
Please note that there are two entrances labeled with this address on Clybourn Ave., and ours is the entrance one door south of the old Relax the Back store.
For entrance into the building, please search for Chicago Psychotherapy or your provider’s name (first initial, last name) in the door directory to be buzzed in. The elevator is located straight ahead once you enter the building, and our office is located on the third floor. The Chicago Psychotherapy office will be just to your right as you exit the elevator on the third floor. Your provider will email you with information about the code to use for the suite entrance keypad. If you have any trouble with the buzzer, please call or email your provider directly.
Parking and Public Transit: If you are driving to your appointment, parking options include metered parking along Clybourn Ave (all hours) or free street parking on neighboring residential streets, prior to permit parking hours. Please do not park in a space designated for another business in the adjacent lot, as the building management will have unauthorized cars towed.
If you are taking public transit, the Armitage ‘L’ stop (brown and purple express lines) is located 2 blocks from our practice, and the North/Clybourn ‘L’ stop (red line) is located approximately 3-4 blocks from our new location.
Once you have set up an intake meeting, we will email you with digital intake paperwork. Please complete these forms at least 24 hours in advance of your intake session. Please email our admin team at [email protected] with any questions.
Payment for services is always due at the time of service, and may be made by credit card (including HSA cards, when applicable). All clients are required to keep a current credit card on file, which will be charged following each billable service, unless an alternate form of payment is provided at the time of service.
Our practice is out-of-network for insurance. At your request, we will be happy to provide you with statements that include all information needed to seek out-of-network reimbursement from your insurance carrier. Out-of-network reimbursement rates vary based on insurance carrier, plan, and service provided. We encourage all clients to contact their insurance plan’s member services department to verify their out-of-network benefits and coverage.
Please see a sample script here as a helpful resource when calling your insurance carrier. Below is a list of the CPT codes most commonly used for psychotherapy, which may be required to check your benefits with your insurance provider:

90791 – Diagnostic Clinical Evaluation (intake session)
90834 – 45-minute individual therapy session
90837 – 60-minute individual therapy session
90847 – Family therapy with the client present
90846 – Family therapy without the client present (includes parent-only sessions for child/adolescent clients)
The pricing for sessions depends on multiple factors, including: session type, session length, provider credentials, and provider experience. Your provider will send you information related to their individual rates for various services as part of the standard set-up process, prior to initiating therapy.
Though we are a self-pay practice, we believe that your income and current life circumstances should not dictate your access to high-quality care. All of our team members reserve a significant portion of their caseloads for income-based, reduced-fee services for individuals who do not have access to seek out-of-network reimbursement with their insurance carrier and have qualifying financial need for reduced fee services. In addition, our Counseling Interns are able to offer substantially reduced-fee services to all of the clients that they serve. If you believe our clinic is a strong fit for you or your family, but have concerns about your ability to pay full-fee costs for our services, please speak with us.
Effective January 2022, you have the right to receive a “Good Faith Estimate” explaining how much your healthcare services will cost. Under the law, healthcare providers (including mental health providers) need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
To avoid a last-minute cancellation fee, please notify us at least 24 hours prior to your appointment to cancel or reschedule. Please note that testing appointments beyond the intake session require a minimum of 3 days’ notice to avoid a late cancelation fee.
We are currently offering in-person sessions to clients who have a preference for being seen in-person. In order to be seen in-person for sessions, clients must be free of any active symptoms of illness, and must be fever-free without the use of fever-reducing medications for a minimum of 24 hours prior to their session. Clients who are currently asymptomatic but have household members with an active COVID-19 or flu infection may only be seen if masked in session. Clients who are unwilling or unable for any reason to abide by these in-person session requirements may be seen via telehealth.
Masking is optional in common areas of the clinic. Masking in sessions will be based on both client and provider preference. Both clients and providers will have the option to un-mask or remain masked in sessions, but are requested to remain masked if it is preferred by either party.
Additionally, all clients with lingering cough or cold symptoms are required to mask while in the clinic common areas and in session.
Clients who arrive early may use the clinic waiting area while waiting for their appointments. However, we ask you to please be mindful to keeping waiting area use before your appointment to 10 minutes or less. If you are a parent or caregiver of a client under age 12, you may opt to use our waiting area while your child is meeting with their provider. To minimize the risk of exposure between clients/families in our waiting area, we ask that caregivers or family members with active symptoms of illness please refrain from using the waiting in the clinic.
The 24-hour cancelation fee will be waived for clients who need to cancel or reschedule sessions due to illness or care of a sick relative, provided you take steps to notify your clinician in advance of your scheduled session that you will need to cancel or reschedule for illness-related reasons. We will do our best to accommodate rescheduling any sessions canceled due to client illness or unexpected caregiving demands. If you become ill on the day of your scheduled session and wish to cancel or reschedule your session, we ask that you please give your provider as much advance notice as possible. Failure to notify your clinician of an illness-related cancelation a minimum of two hours in advance of your scheduled session will result in a late cancelation or no-show fee.
If you or someone in your household is currently symptomatic with COVID-like or cold/flu-like symptoms, we kindly ask that you reschedule your appointment to telehealth or for a later time, even if you test negative for COVID-19 and flu. Any clients or caregivers experiencing fever must remain fever-free without the use of fever-reducing medication for a minimum of 24 hours before returning to the clinic in-person. Clients experiencing GI upset (e.g., vomiting or diarrhea) must also be symptom-free for a minimum of 24 hours before returning to the clinic in-person.

FAQ
YES! We know that the current mental health crisis has resulted in significant wait times for folks trying to access care, and we are doing our best to keep up with the demand. Availability and wait times typically depend on provider fit, your scheduling flexibility, the need for in-person versus telehealth-based services, and preferred location. Clients who are a good fit for our services can typically be seen for an intake session within 1-3 weeks. Those who require after-school/after-work times and/or in-person care may experience longer wait times, depending on provider availability. If we believe that our wait time for your particular needs will be longer than a few weeks, we will be sure to provide you with additional referrals as well.

We support clients across the lifespan (ages 3+) with a range of support needs. Our practice is out-of-network for insurance, meaning that clients pay out-of-pocket for services at the time of service. Upon request, we can provide clients with Superbill statements following each clinical service provided. The Superbill statement contains all of the information necessary to submit to your insurance provider for any out-of-network reimbursement offered by your health insurance plan. We also offer income-based sliding scale fees, to help mitigate out-of-pocket expenses to those who qualify. Our clinical interns offer reduced-fee services to all clients that they serve, and can see clients for between $30-75 per session, depending on client income.
The pricing for sessions depends on multiple factors, including: session type, session length, and provider experience and credentials. In general, our standard 45-50 min session rates for licensed, post-graduate providers range between $145-$215/session, depending on the provider, their training background, and years of experience in the field. Our clinical interns are available to see clients for as little as $30-75/session, depending on client income and household size.
There is no charge for our initial phone consultation for new/prospective clients. During this call, a member of our team can answer any questions you have about cost and payment for services provided by our providers, including your eligibility for sliding scale or reduced fee services.

Each insurance company will have a different out-of-network reimbursement submission process. However, most use a standard claim form that can be filled out and submitted alongside your Superbill statement(s) as supporting documentation for your claim. Covered costs for services are typically applied toward your plan’s out-of-network deductible and once this out-of-network deductible is met, many plans will reimburse a certain percentage of the allowable amount for covered clinical services rendered.

The length and frequency of sessions will be mutually decided between you and your provider, during or following the intake session(s). The initial 1-2 intake sessions typically last 60-75 minutes each. Thereafter, individual therapy sessions typically last approximately 45 minutes and clients usually meet with their provider on a weekly basis, at least at the outset of therapy. Based on a number of factors, some clients may opt for longer (e.g., 60-minute or 75-minute) sessions, and others may opt for shorter (e.g., 30-minute) sessions. Once a client meets or mostly meets their initial therapy goals, session frequency may be adjusted to bi-monthly or monthly appointments, considered “treatment maintenance” sessions.
Most of our providers offer both in-person and virtual therapy options. Clients for whom in-person sessions are preferred may be seen in-person by mutual agreement between the clients (or caregivers) and providers.
Not all of our providers offer in-person sessions, and in-person appointments may be limited to certain days for those who do see clients in-person, so availability of in-person sessions will depend on which provider in our group is the best fit for your care. If you have a strong preference for in-person sessions, please let us know this during your initial consultation call. Clients wishing to be seen in-person must be willing to abide by our current illness policy. Clients who are unwilling or unable to abide by our illness policy for in-person sessions may be seen remotely via telehealth only.
Please note that mental health licensing is state-specific and clients must be physically located in IL at the time of sessions, unless your provider has specifically indicated that they are authorized to see you in another location.

In the event that we do not have a good fit available for your care, we will provide you with outside referrals and/or put you on a waitlist for our next available strong fit.
